At Made to Help, our work focuses on helping each participant navigate and thrive within their unique journey. This progress report highlights the individual achievements of several participants, along with the robust support strategies we have put in place to manage risks and promote well-being. Each participant’s journey reflects our commitment to fostering autonomy, building social connections, and ensuring a safe, nurturing environment.


Participant Achievements and Growth

Building Social Confidence through Community Engagement

Participant A’s journey has been one of growing social confidence and community involvement. Together with A’s support worker, A has embraced various community activities, including weekly bingo, fitness sessions at the gym, and group outings. This supportive companionship has been instrumental in helping A feel more connected and socially engaged. Beyond just participating, A is forging meaningful connections and finding comfort in social settings that were once daunting.

Living Independently with Comprehensive Support

For Participant B, independence at home is both a goal and a reality, supported by family members and a team of caregivers. Home modifications, including a newly accessible bathroom, have enabled B to remain safely at home. Through constant monitoring and tailored support, we aim to provide a stable and reassuring environment for E.

Participant F has a series of unique health needs, which we address through careful planning and targeted interventions. Dietary adjustments, tailored assistive devices, and customized behavioral strategies ensure that F can engage in daily activities safely and comfortably. For example, a specialized meal plan and adaptive equipment aid F in managing their nutrition and physical well-being. The aim is to provide F with the tools needed to navigate everyday life confidently and independently.

For Participant G, maintaining health and safety requires close collaboration among allied health providers and the support coordination team. Community Engagement: We have facilitated opportunities for our participants to engage with their communities, whether through social activities, exercise programs, or community participation. This helps them build social networks and improve their overall quality of life.

Safety and Wellbeing: Ensuring the safety and wellbeing of our participants is our top priority. We have implemented various measures to support them, including 2:1 support for self-care, mobility, and eating, as well as behavior management strategies to address any challenging behaviors.

Future Planning: We are committed to helping our participants achieve their long-term goals. This includes finding suitable housing and exploring employment opportunities, as well as securing appropriate accommodation and ensuring they have the necessary supports to manage their conditions.

Participant A:

  • Engagement with Mental Health Supports: Successfully connected with neuro-divergent identifying psychologists and allied health professionals.
  • Physical Health Improvement: Regular appointments with local occupational therapists and physiotherapists have been scheduled.

  • Communication Skills: Uses an iPad and Pro To Go application to express needs effectively.
  • Diet and Nutrition: A new dietician has been arranged to provide advice on reducing weight and improving diet.
