Emergency Readiness
Use this form with your participant to plan for emergency situations. Make sure all individual support networks have a copy once complete. Share this with the participant and ensure careflo makes it available to support workers.
FIRST VISIT - Home Safety Checklist
Home visit risk assessment introductory statement (Can be read out over the phone to explain why we need to do the risk assessment) MTH has a duty of care to make sure that all staff are safe at work – so it is routine for us to ask some questions before we visit you at home.

The information assists MTH to tailor our support services to meet the participant’s individual needs. Please complete all sections of the form. NOTE: A copy of this Support Plan is given to the Disability Support Workers assigned to work with the Participant.
Transistions
Made To Help Participant Transition Risk Identification and Response Instrument. This instrument is to be completed bythe provider from which the participant is transitioning. The primary objectiveof this tool is to pinpoint and document potential risks associated with eachtransition to or from the provider. This encompasses not only typicaltransitions but also temporary shifts that may occur in response to specificrisks faced by the participant, such as instances necessitating hospitalizationdue to healthcare concerns. The aim is to comprehensively identify, document,and respond to these risks to ensure the participant's safety and well-beingthroughout the transition process.
Vulnerability Assessment
The NDIS Practice Standards advise providers to implement strategies that prevent conflicts of interest, which can include ensuring that no single worker holds excessive influence over a participant's care.
